Essential information

· Patient name, DOB, NHS number
· GP or optometrist name and contact details
· Chief complaint and duration
· Relevant ocular history
· Current systemic medications
· Visual acuity (if assessed)
· Relevant examination findings

Recommended for complex cases

· Recent fundus photographs or OCT reports
· Previous ophthalmic clinic letters
· Existing glasses prescription
· Vision screening results for children
· Relevant systemic conditions (diabetes, hypertension)
· Current anticoagulant or antiplatelet therapy
